I hereby apply for membership in Pine Point Ski Club for the membership year identified above, which runs from September 1 to August 31 each year. In consideration for club membership and the opportunity to attend club trips and club activities, the undersigned agrees to and hereby does discharge and release: (a) the Pine Point Ski Club, and (b) its officers, agents, representatives, and trip leaders in connection with the performance of their duties on the club’s behalf, from and against all claims for injury to the person or property of the undersigned, unless willfully and maliciously caused. I certify my age to be 18 years or older.

Membership dues are nonrefundable and are not tax-deductible, according to the IRS. 

I acknowledge and agree that this document may be executed by electronic signature, which shall be considered as an original signature for all purposes and shall have the same force and effect as an original signature, without limitation.
